Whether for post-acute care patients or long-term care residents, urgent problem-focused assessments or longitudinal care with regular follow-up; we offer consultative services right at the bedside, making it fast and easy for your staff to setup cardiology services in a timely manner.

Post-acute care patients often have complex medical problems and recent changes to their medications, all of which need continual monitoring. Unfortunately, it is not uncommon for a patient to be discharged from post-acute care just to be readmitted to the hospital weeks later. In an effort to improve patients’ health, we continue caring for patients transitioning back home, providing home visits to reassess their medical condition and treatment.

With Hearts at Home, patients no longer need wait weeks or months for a preop exam before elective surgery. We assess patients at the bedside and perform routine preoperative cardiac diagnostic testing at the point of care.

We assist your clinical staff by providing noninvasive cardiac diagnostic testing onsite for fast results.

We take pride in our outstanding communication with all facility staff, both medical and clerical. We provide visit notes the same day of service and communicate new recommendations directly to medical staff, we call medical providers for concerning findings requiring urgency, and we are always available via direct phone line, text, and email.

You can reach us by phone, text or email to enquire about how Hearts at Home can provide solutions to fit your needs with no additional cost to you. We can then schedule a meeting with facility staff to further discuss our services. If Hearts at Home is a good fit, we can set a start date pending a signed contract and completed onboarding with the facility and electronic medical records.

We typically arrive at the facility early-mid morning depending on what is convenient for your staff. We then meet with the nurse supervisor and/or medical provider to go over the list of patients needing to be seen that day at which time any additional patients (e.g., new admissions, hospital follow-ups) can be added on if needed. We then interview and examine patients at the bedside or in another private area. If there are any immediate concerns we discuss them with the nurse/provider before leaving the building. We complete our chart review and documentation for each patient seen offsite, send/upload visit notes to the facility, and reach out to medical staff with any new recommendations.

It depends on the needs and the size of each facility. We typically visit a facility every 2 weeks. For facilities with fewer patients we may decide together to extend the visits to monthly. For those facilities caring for a large number of patients we may visit weekly or even more depending on the need.
Because unpredictability is always a factor, we may be able to schedule a same or next day visit for urgent matters.

We bill insurance carriers and do not charge facilities for our regular services.
